I have also experienced inconsistent behavior with the Quick Folder Key Navigation addon. Once I switched to version 3 of this addon I could not get the addon to function once I closed and restarted Thunderbird. I even removed and reinstalled the addon but this did not fix the problem. I even contacted the author of the addon but he was not able to duplicate the problem. Once a new Thunderbird update was downloaded the addon seemed to function inconsistently but then shortly after the update was installed it began to function consistently and worked every time I started Thunderbird. However, a subsequent update broke this consistency and now the addon once again stops functioning as soon as Thunderbird has closed and then restarted. I then have to disable and enable the addon in order for it to function but it only does so during the current session.

I said this to Gene privately recently, but since it's come up here, I do not have this problem, many other people I know do not have this problem with the latest versions of Thunderbird and the Quick Folder Key Navigation Add-On.
When I open the Thunderbird Add-Ons manager and hit the triple-dot menu button for Quick Folder Key Navigation and go to its options, I do see something "odd" that I can't explain why it's this way, but it's worth checking. There are two checkboxes in the Options, one clearly labeled, "Enable Mail-folder key navigation," along with a second, unlabeled checkbox that is checked. I have no idea if something will be announced beyond "checkbox" for that second checkbox, but mine's checked and the issue described with constant need to disable/reenable is not occurring, and that's over a long period now.
The description in the add-on manager also goes into further detail than it once did, and I'll put the text here for easier review:
